1. Details given in the Application Form will be truthful and accurate.
2.

You have carefully read the “scheme’s benefits, terms & conditions”, “limitations” & “exclusions” relating to the scheme and have understood the same.

3.

In case you are given an appointment at the hospital you will maintain punctuality of the date & time of the appointment.

4. When you discuss your problems with the Doctor, you will give full and accurate information.
5.

You will strictly follow all the instructions given by the treating doctor like doses, medicines, diet or any other restriction.

6.

While you are under the treatment of a particular Doctor at Hanumant Hospital, you will not take any medicine prescribed by any outside Doctor without permission of your treating doctor.

7.

In case of hospitalization, you will follow all instructions given to you faithfully and will maintain cleanliness.

8.

You will not bring any outside food inside the hospital either during OPD consultations or hospitalization.

9.

Pan / masala / gutka chewing & smoking is strictly prohibited in the hospital.  You will strictly comply with this during your OPD/indoor visit.  Pan / masala / gutka / bidis / cigarettes will be deposited at the entrance before entering the hospital.

10.

In case you are taking any medication prescribed by the doctor and experience any side effects, you will report the same immediately to the concerned doctor at the hospital.

11.

You will not demand any service or medicine at the hospital, which is outside the scope of the scheme.

12.

You and your relatives will deal with all the hospital staff & the doctors in a courteous & respectful manner. 

13.

In case you have any complaint, the same may be given in writing or communicated verbally to the Administrator Sanjeevani Scheme.

14.

You will ensure that your relatives will visit only during the visiting hours.  No children will be allowed to stay with the patient during hospitalization. 

15.

No relatives will be allowed to sleep in the hospital wards without doctor’s advice.  They can use the dormitory facility provided by the hospital on paying normal charges.

16.

Use of mobile phone is prohibited in the indoor area of the hospital as well as OPD chamber and investigation wing.  You will strictly comply with this.

17.

The jurisdiction for settling any disputes / differences / suites against the Sanjeevani Lok Arogya Scheme shall lie within the legal jurisdiction area of Mahuva Taluka, Gujarat State.
